YOUR HOME. YOUR COMMUNITY. YOUR CHOICES.

Individual Community Living Support

Practical support for older adults to keep living life in their own home.

Everyday support starts with the person’s own routines and goals.

INDEPENDENCE, WITH A LITTLE SUPPORT

Keep your routines.
Stay connected to your life.

Individual community living supports (ICLS) combine scheduled assistance, reminders, cues, and support to help eligible older adults remain in their own homes.

The starting point is the life you want to keep living. Tell Alliance what is working well, where daily tasks feel harder, and the people, places, and routines that matter to you.

WHO IS ICLS FOR?

A clear starting point.

ICLS serves eligible adults age 65 and older enrolled in Minnesota’s Alternative Care (AC) program or Elderly Waiver (EW).

The plan must include at least two of the six support areas. Your case manager or care coordinator completes the ICLS planning form with you.

Not sure whether this is the right service? Share your situation with us, and ask your care coordinator about the options.
