SUPPORT FOR LIVING AT HOME
Meaningful company.
More confidence in everyday life.
SUPPORT BUILT AROUND YOUR GOALS
01
Conversation and shared activities can help an older adult work toward a meaningful goal in their support plan.
02
A companion may join the person at a community event or activity while supporting their participation goals.
03
Verbal guidance and cues can help the person take part in tasks connected to their agreed goals.
Adult companion services are non-medical. They do not include hands-on nursing or assistance with personal-care activities such as bathing and dressing. Ask us which service fits your needs.
YOUR INTERESTS. YOUR CHOICES.
Tell us about the people, places, and activities that matter to you. A useful conversation can begin with what you enjoy, where you would like more connection, and what makes support feel comfortable.
Family members and care coordinators can join the discussion while keeping the individual’s preferences at the center. Your case manager can help review service eligibility and authorization.
Interests and preferred activities.
